 Request for Use of District Facilities


Thank you for using the online version to expedite the facility use process.

To request use of District Facilities:

1) Complete the online form.
2) Print and sign the form.
3) Take the form and your Certificate of Insurance to the building you wish to use.

If you do not have access to a printer, a blank form can be obtained at Central Office or any school building during normal hours.

Please read all regulations carefully and check the charges prior to completing the form. Requests must be made at least seven business days prior to the time of requested use. You will be notified by mail when your form has been approved. Please direct any questions to the office at the building you wish to use:
